This form is only for customers who want to cancel an order before it has been shipped. If your order has already been shipped, delivered, or received, please do not use this form. In that case, you must submit a Refund / Return Request instead. Cancellation requests submitted after the order has already shipped may not be accepted.

This form is for customers who want to request a refund or return after the order has already been shipped or delivered. You should use this form if you received the product, if the product arrived damaged, if you received the wrong item, or if you want to request a refund for an order that is no longer eligible for cancellation.
